We strive to provide our patients, not only the best dental care but also include the best clerical support staff. As part of our goals we have reduced the patient paperwork packet from 8 pages to 4 pages for 2014. The packet includes basic patient contact information, along with spouse or guardian contact information, and primary dental insurance information on the first page. Patients who are covered under multiple dental insurances or who also have items being submitted to medical insurance will need to fill out one additional form, the multiple insurance form. We ask that patients update the first page on an annual basis just to insure that address changes and insurance changes have not taken place.
The Second page of the patient packet covers medical health history for the patient and needs to be updated every year. When any change in health history occurs we ask the patient to notify the practitioner to insure everyone is knowledgeable of the changes. The final two pages cover our financial policies and acknowledgment of our HIPAA Policies. Our Policies are easily accessible here HIPAA.
